After booking an appointment
If you are new to the practice and have made an appointment with our GP, please download and complete the registration form. Once complete, please email the form (PDF format) to email@example.com prior to your appointment (including telehealth bookings).
If you booked online via HealthEngine, please select the link in your booking confirmation email to complete the online new patient registration form.
Please note, under new Medicare COVID-19 telehealth restrictions, a new patient is eligible for telehealth only if they have an existing COVID-19 infection.
COVID-19 Vaccine Boosters
BOOSTER VACCINATION (3rd Dose):

All individuals aged 16-17 years can receive a third dose of Pfizer Comirnaty COVID-19 booster vaccine.
All individuals aged 18-29 years can recieve a third dose of either Pfizer Comirnaty or Moderna Spikevax COVID-19 booster vaccine.
BOOSTER VACCINATION (4th Dose):
All individuals aged 30+ years can receive a fourth dose of Pfizer Comirnaty or Moderna Spikevax COVID-19 booster vaccine.

COVID-19 VACCINE SIDE EFFECTS
Flu-like symptoms are common following any COVID vaccinations. Side effects include but are not limited to:
- Pain at the injection site
- Muscle pain/bodyache
- Joint pain

You are eligible for Spikevax (Moderna) COVID-19 vaccinations if you are aged 12+ years. Click here for more information.
You are eligible for PFIZER COVID-19 vaccinations if you are aged 12+ years. Click here for more information.
You are eligible for Paediatric PFIZER COVID-19 vaccinations if you are aged 5-11 years.
GETTING PROOF OF VACCINATION
You can log onto your myGov account to get either:
an Immunisation History Statement - this shows all the immunisations you’ve had that are on the Australian Immunisation Register (AIR).
COVID-19 digital certificate - this only shows your COVID-19 vaccinations. You’ll be able to get your digital certificate after you’ve had all required doses of a vaccine as approved for use in Australia.
The GP provider needs to input your vaccination details to the AIR so you can get proof. This can take a few days.
