COVID-19 Vaccinations

 

NO PAPER FORMS ACCEPTED

All necessary patient forms and related links are listed below. No online bookings for COVID immunisation. Please ring our reception team to make vaccination bookings. If we don't answer your call during operating hours, it is likely that our staff are busy with calls. Please allow about 15-20 minutes before trying again. 

BEFORE MAKING YOUR FIRST BOOKING 

Please check your eligibility before contacting us. 

PRIMARY VACCINATIONS (1st and 2nd Doses):

You are eligible for Pfizer or Moderna COVID-19 vaccination if you are aged 12+ years. 

BOOSTER VACCINATION (3rd Dose):

All individuals aged 18+ are eligible for a Pfizer or Moderna COVID booster vaccination five months after their second dose. 

Severely immunocompromised individuals aged 18+ can receive their booster dose at least two months after their second dose.

 

At current ATAGI has not recommended booster doses for individuals aged under 18 years of age.

AFTER MAKING YOUR FIRST BOOKING 

For all patients who have booked for a COVID vaccination with us, you must download and complete the COVID consent form and send it back to us via email. If you are are new patient, you will also need to fill in the new patient form. 

 

You must complete ALL relevant forms and email the paperwork to us via reception@rtmpt.com.au 24 hours prior to your vaccination appointment. If the paperwork is incomplete or not sent to us beforehand, you will not be able to proceed with vaccination even if you have a booking.

AFTER HAVING YOUR FIRST DOSE

After your first COVID vaccination, please book your second dose with either our nursing staff or reception team before you leave the practice. Please monitor yourself for side effects. If you encounter any major side effects, please book a telephone consultation to speak with one of our doctors. All major side effects will be reported to Public Health and/or TGA.  

 

COVID-19 VACCINE SIDE EFFECTS

Flu-like symptoms are common following any COVID vaccinations. Side effects include but are not limited to:

- Pain at the injection site

- Fever/chills

- Headache

- Tiredness/lethargy

- Muscle pain/bodyache

- Joint pain

GETTING PROOF OF VACCINATION

You can log onto your myGov account to get either:

  • an Immunisation History Statement - this shows all the immunisations you’ve had that are on the Australian Immunisation Register (AIR).

  • COVID-19 digital certificate - this only shows your COVID-19 vaccinations. You’ll be able to get your digital certificate after you’ve had all required doses of a vaccine as approved for use in Australia.

 

The GP provider needs to input your vaccination details to the AIR so you can get proof. This can take a few days.
