Due to the overwhelming influx of calls and phone enquires, we have detailed necessary information regarding our COVID vaccination procedure below. Please read the following information before calling us.
NO PAPER FORMS ACCEPTED
In order to protect patients and staff, our practice is strictly going paperless in order to reduce direct contact and lower any risk of transmission. All necessary patient forms and related links are listed below. No online bookings available for COVID immunisation. Please ring our reception team to make bookings. If we don't answer your call during operating hours, it is likely that our staff are busy with calls. Please allow about 15-20 minutes before trying again.
BEFORE MAKING YOUR FIRST BOOKING
Please check your eligibility before contacting us.
You are eligible for AstraZeneca COVID-19 vaccination if you are aged 18+ years.
You are eligible for PFIZER COVID-19 vaccination if you are aged 12+ years.

AFTER MAKING YOUR FIRST BOOKING
For all patients who have booked for a COVID vaccination with us, you must download the COVID consent form, complete it and send it back to us.
If you are are new patient, you will also need to fill in the new patient form and send us a copy of your patient health summary from your regular GP (if you have one).
If you are 18-59 years of age and will be getting the AstraZeneca COVID vaccine, you must also sign the COVID AstraZeneca waiver form.
You must complete ALL relevant forms and email the paperwork to us via firstname.lastname@example.org 24 hours prior to your vaccination appointment. If the paperwork is incomplete or not sent to us beforehand, you will not be able to proceed with vaccination even if you have a booking.

AFTER HAVING YOUR FIRST DOSE
After your first COVID vaccination, please book your second dose with either our nursing staff or reception team before you leave the practice. Please monitor yourself for side effects. If you encounter any major side effects, please book a telephone consultation to speak with one of our doctors. All major side effects will be reported to Public Health and/or TGA.
COVID-19 VACCINE SIDE EFFECTS
Flu-like symptoms are common following any COVID vaccinations. Side effects include but are not limited to:
- Pain at the injection site
- Muscle pain/bodyache
- Joint pain

If you have booked for an AstraZeneca COVID vaccination, please refer to the instructions outlined above.
You are eligible for AstraZeneca COVID-19 vaccinations if you are aged 18+ years. Two doses are required to complete the vaccination course. The second injection can be given between 4-12 weeks after the first injection.
If you are under 60 years of age and will be receiving the AstraZeneca COVID vaccine, please sign the COVID AstraZeneca waiver form. GETTING PROOF OF VACCINATION
You can log onto your myGov account to get either:
an Immunisation History Statement - this shows all the immunisations you’ve had that are on the Australian Immunisation Register (AIR).
COVID-19 digital certificate - this only shows your COVID-19 vaccinations. You’ll be able to get your digital certificate after you’ve had all required doses of a vaccine as approved for use in Australia.
The GP provider needs to input your vaccination details to the AIR so you can get proof. This can take a few days.
